||Description: UF Health, the medical network associated with the University of Florida, recently launched a comprehensive quality-of-care project known as EEPC (the Efficiency, Effectiveness and Patient Centeredness initiative) which focused on many different domains of care delivery including admission/geocentricity, case management and discharge placement, discharge appointments, hospice and palliative care, care redesign, community engagement and patient centeredness.
The University of Florida Office of CME participates on the overall EEPC steering committee and several of its domain working groups, but also has a history of running individual QI projects at their affiliated group practices, often with in-kind commercial support. Within the new environment, the UF CME office had to plot a course for aligning its QI work within the overarching EEPC framework.
The UF Office of CME is directing an ongoing, commercially-supported project called iHEART (Improving Heart Failure Admission and Readmission Together) which is serving as a therapeutically-specific branch of the larger EEPC mission. This session will use the iHEART project as a case study for how QI projects can, and should, align with overall quality initiatives within a health system in order to satisfy multiple stakeholders and improve performance against a variety of outcome, cost and satisfaction measures.
